About
Each person who is engaged in the business or installing or offering to install radiation machines or is engaged in the business of furnishing or offering to furnish radiation machine servicing or services in this state shall apply for registration of such services with the agency prior to furnishing or offering to furnish any such services.

Who Applies Any person or facility offering radiation machine services including furnishing, selling, calibration, installation, service or repair and radiation protection or health physics consultations.
When To Apply Prior to providing service.
Initial Requirements Complete online or paper application. Include training and or experience to perform services.
Duration One year.
Processing Time 30 days.
Fee Structure Application: $200.
Renewal Requirements Registration fees due yearly.
Exam Does not pertain.
